What if I need services for someone else?

My Primary Care works with many diverse populations and understands that not every shoe fits when it comes to health care. Our team can work to keep you involved in the care of your loved ones or conservatee through regular updates and follow-ups.

How is Mobile Primary Care different?

Mobile primary care reduces travel barriers, lowers ER use, and improves access for older adults and those with mobility or transportation limits. Appointments are scheduled for convenience, with a focus on keeping patients healthy in their own homes.

What areas do you service?

My Primary Care serves most of San Diego County and may already be a provider in many facilities in your area. Whether you live in an independent living facility, board and care, assisted living facility, dementia care facility, or a private residence, let My Primary Care bring care to you!

What if I have prescriptions from another provider?

As part of our commitment to your care, My Primary Care reviews every patient’s medication list and ensures the necessity of all current medications and may be able to offer refills for medications prescribed by your specialist.
